1. Open the form to convert your Facebook account

2. Complete the form
Next, you must complete the form with:
- The first and last name listed on the deceased's Facebook account
- The date of death
- Proof of death (obituary, memorial card, etc.)
- Your email address

3. Submit the form
At this point, simply click the "Submit" button in the bottom right corner of the form. Facebook will then contact you to confirm your identity and provide supporting documents.
